You are working the night shift on a medical-surgical unit. Your assignment includes a 19-year-old woman
admitted early this morning. She has sustained burns to 30% of her body surface area.
1. The staff are following the Parkland Formula for fluid resuscitation. The client's injury occurred at
0200, and treatment began at 0400. She weighs 110 pounds. Calculate her fluid requirement
using the Parkland formula. Explain the time intervals and amounts for each (mL/hr).
A patient that is 110 pounds is 50 kg, the parkland formula would be 4mL x BSA % x weight
(kg). For this burn patient, the formula would be 4ml x 30% x 50kg. The total amount of
fluid the patient would need in 24 hours would be 6000mL. The total amount of fluid is then
divided by two which is 3000. The patient came to the hospital 2 hours after the burn occurred so,
the first amount of fluid needs to be given with 6 hours and the second half will be
given within 16 hours. The first amount of fluid is 500mL/ hr and the second amount
would be 187.5mL/ hr, totaling 6000ml in 24 hours.
4mL x 30% x 50kg = 6000mL
6000mL / 2 = 3000
hours (when the burn first occurred)
3000/ 16 hours (remaining fluid to be finished in 24hours)
3000mL / 6hours = 500ml/hr
3000mL/ 16 hours = 187.5mL /hr
2. The client was sleeping when the fire started and managed to make her way out of
the house through thick smoke. You are concerned about possible smoke inhalation.
