Hi, | just put Josie Brennan in a room. | got her history. She is 15 years old and came to the clinic
with her mom for concerns of fatigue, dizziness, and palpitations after her past few cross-country
track meets. Her season just started 3 weeks ago at the beginning of summer. Overall, she is
healthy and does well in school and other extracurricular activities. She has never experienced this
fatigue before correlated with strenuous exercise and does not know why this is happening to her.
She performed well in cross-country last year but has struggled to keep up with her friends this
season. She does not have any history of significant chronic health problems. | put her vital signs in

6/20 Nursing Note: Came to clinic with fatigue, dizziness, and palpitations
1600 during her past few cross-country track meets. She was able to run about 1
% miles, but after that, her symptoms arose, and she could not finish the
races. She notes she is often more short of breath after running than usual.
After resting for 20-30 minutes, her symptoms are alleviated. Her season
started 3 weeks ago. Last year, was able to run 5-6 miles and keep up with
her friends with no problem. Denies any recent illnesses, injuries, or chest
pain. States she has been more irritable lately. Her period has been a
regular 28-day cycle with no change in menstrual flow. Notes that she drinks
at least 68 ounces of water every day, eats a healthy vegan diet with plant-
based protein, and exercises frequently. Started the vegan diet because she
wanted to eat healthier and did not enjoy the taste of animal products.
