the requirements for this course. These objectives should be aligned specifically to your Practicum
experience. Your objectives should address your self-assessment of the skills found in the “PMHNP
Clinical Skills Self-Assessment Form” you completed in Week 1.
As you develop your individualized practicum learning objective, be sure to write them using the
SMART format. Use the resources found in Week 2 to guide your development. Once you review

Objective 1:
I want to improve my clinical skills in conducting comprehensive psychiatric evaluations for adult
and pediatric patients.
Planned Activities:
Demonstrate an ability to accurately diagnose and formulate treatment plans for 90% of both
adult and pediatric patients, as appraised by supervisors and gauged against DSM-5 criteria.
Engage in 10 psychiatric evaluations under supervision and conduct a monthly
selfassessment to track personal progress.
Mode of Assessment:
Ensure that all relevant information is accurately entered and stored in the Meditrek system,
and that it is also included in the assessment of the preceptor.

• Develop differential diagnoses for patients at all stages of life within mental health care
