A. Health History

E.H is a 54-year-old Caucasian female. She weighs 116.12kg and she is 172.72cm. E.H

was seen in the ER with complaints of left foot, left hand, and right shoulder pain. She stated

that she twisted her left, but is not sure how. Upon arrival, she was hypotension, tachycardia,

and hypothermic. In further conversation the nurse found out that she has an history of alcohol

abuse which lead to her diagnosis of cirrhosis of the liver. It also explained the distended

abdomen and pitting edema. E.H never admitted or denied being intoxicated at the time of the

injury. She arrived to the ER by EMS with no family or friends accompanying her. Her marital

status and family support is unknown.

B. Laboratory/Diagnostic Testing

The diagnostic tests ordered included a CBC, Chemistry, Point of Care testing, Lactic

Acid, and an MRI of the cervical spine and forearm, which was not completed because the

patient stopped mid procedure. The reason that they performed a CBC, Chemistry, and Point of

Care testing is because this is standard to see where all of the values are. The lactic acid level

was drawn because of the suspected sepsis and it was high at 3.56 (should be below 2). The

MRI of the cervical spine was done because of neck pain and the MRI of the left forearm was

done because of pain. PTT and INR was done and the PTT was 53.0 and the INR was 4.49. These

are both high levels and indicate that this patient is at a high risk for bleeding, which is

consistent with the diagnosis of liver cirrhosis. WBC count is high at 18.8 which is partly because

of the liver cirrhosis and also because of the sepsis. The BUN (43) and Creatinine (2.0) are both

high, likely due to the fact that she is in liver failure and the kidney is starting to take over for

the liver malfunctioning.
