Racial and ethnic minorities have greater health disparities that predispose them to

chronic conditions. Among these populations, poor control of chronic illnesses is evident which

also leads to increased cases of complications (American Diabetes Association, 2017). This

paper presents a PICOT question evaluation that focuses on the poor control of diabetes in

Blacks.

The population of focus is the Blacks. Compared to their white counterparts, Blacks have

higher incidences of poorly controlled diabetes. This contributes to poor health outcomes,

increased complications and poor quality of life. Black patients hardly meet their treatment goals

or adhere to their treatment guidelines (American Diabetes Association, 2017). In this regard, the

picot aims at finding out what interventions can be put in place to address the racial disparities.

The major causes of disparities responsible for poor management of diabetes are poor

access to care, loss of follow up visits, poor drug adherence, improper dieting and lack of

exercises. Consequently, patients are unable to maintain their glycemic levels within the

recommended range or meet their glycemic goals (Schmittdiel et al., 2017). This increases their

risks of developing secondary complications associated with diabetes such as neuropathies,

opportunistic infections, and glaucoma.

Numerous interventions have been put in place in a bid to reduce the racial disparities

and improve outcomes. Determining the impact of the interventions on reducing the health

disparities is essential in this case. One of the interventions that have been used on diabetic

patients is the use of electronic health record (EHR) system in the management of diabetes.

The EHR system can be used to improve diabetes treatment and management by use of

diabetic registries and clinical decision support system. These EHR tools enhance the
