Mr. Carter has taken two medications since he has been diagnosed. The medications are Olanzapine and Vanlafaxine. 4. Identify and document key nursing diagnoses for Mr. Carter. a. Distrubed Sensory Perception: Auditory/Visual r/t altered sensory perception AEB vocalizing “The Spies” and that everything has poison. b. Impaired Social Interaction r/t/ difficulty concentrating secondary to auditory hallucinnations AEB no response to some questions and no direct eye contact. 5. Referring to your feedback log, document all nursing care provided and Mr. Carter’s response to this care. a. Mr. Carter was made sure that he was in a safe environment b. Lorazapam 2mg was administered c. A series of mental health questions were asked d. A Mental Health Examination was performed and given to the charge nurse as instructed. 6. Document your handoff report in the SBAR format to communicate the care plan for Mr. Carter to the nurse on the next shift. Situation: David Carter is a 28 yr old male who was brought into the ER by the police. He became violent towards his mother when asked to take his medications Background: David was diagnosed with Schizophrenia at 18 years old. He has taken his medications and has been stable. He stopped taking his medications because the Auditory Halllucination’s (voices) are telling him that there is poison in everything. His speech is tangential and he is unable to answer some questions. He has refused to shower and appears dirty. Assessment: A Mental Status Examination was performed. Therapuetic responses and empathetic responses were to used to orent him to reality. He is less agitated than at first and he agreed to eat if what he is eating is prepackaged. Recommendation: Mr. Carter needs to change his medication where he is not gaining weight. Mr. Carter should continue to be oritented to reality, but be reassured that his paranoid delusions are invalid by providing packaged food for the time being.
