Nsg 530 Study Guide Exam III 2026-2027
module 9-12 questions and verified
answers

Module IX - Respiratory Disorders
Asthma - For the purpose of this course, consider only those 12 years of age or older
and adults
• Classification of medications, adverse effects, drug interactions, contraindications, monitoring
and patient counseling are always essential
• Have you reviewed the learning objectives, "Weekly Recap" and "Fuel for Active Learning"
• What are the major pathophysiological characteristics associated with asthma and how does
that relate to the therapeutic approach to disease
• What are the precipitating factors / triggers associated with asthma (pg. 243 and case study) o
Recall discussion regarding ACEIs and beta-blockers (CV module)
• After a diagnosis is made, what factors are used to determine the components of severity
(graphic I provided and table 14-1)
o Impairment
Symptoms: night awakenings, need for SABA, work/school days missed, ability
to engage in normal activities
Lung function: spirometry (FEV and FEV1/FVC except for children 0-4 yo)
Risk
o Exacerbations
A flare-up or exacerbation is an acute or sub-acute worsening of
symptoms and lung function compared with the patient’s usual status

• What are quick relief (rescue) medications? Describe their mechanism of action and proper
place in therapy o All patients with asthma (intermittent or persistent) should have SABA
available for PRN use
• What are long term (controller) medications? Describe their mechanism of action and proper
place in therapy o What is theophylline current place in therapy (Asthma / COPD) and why has
it fallen out of favor ?
Consider the many drug-drug, drug disease interactions
• Based on components of severity, determine the recommended step for initiating therapy and
PREFERRED therapy for that step (table 14-1. Figure 14-2 or the graphics I provided)
• What are the components of control and how and when should therapy be adjusted (i.e. Step
up or step down) o Always consider I.C.E.
• How should exacerbations be managed and what is the importance of patient education and
having a written action plan
COPD
• Classification of medications, adverse effects, drug interactions, contraindications, monitoring
and patient counseling are always essential
• Have you reviewed the learning objectives, "Weekly Recap" and "Fuel for Active Learning"
• Have you looked at the "Additional Cases" and "Answers" post
• How does the pathophysiology of COPD differ from that of asthma and how does that relate to
the therapeutic approach to disease o Spirometry is required to make a clinical diagnosis of
COPD; the presence of a postbronchodilator FEV1/FVC < 0.70 confirms the presence of
persistent airflow limitation and thus of COPD.
• What re some risk factors associated with COPD and how / why should providers encourage an
active reduction in risk factors
• Consider the combined assessment of COPD o How is the GOLD classification system,
symptoms scores (CAT, mMRC) and risk for exacerbation used to determine appropriate
therapy for a patient?
o Identify a patient as A,B,C,D based on these criteria (also table 15-2)
None of the existing medications for COPD has been shown conclusively to
modify the long-term decline in lung function.
