Basic extraction skills station - recommended approach - ANSWERYou are

1. PPE - mask and visor, wash hands, then gloves (must use correct order)
2. Select correct forceps for tooth AND handedness
3. Patient positioning
4. Operator positioning
5. Hold forceps correctly - from underneath and curve of handle resting in
palm of hand
6. Apply forceps correctly - blades applied bucco-lingually into gingival
crevice, vertically down long axis of tooth
7. Remove fingers from between handles
8. Position supporting hand correctly
9. Use correct movement for forceps
10. Extract correct tooth
Forceps for extractions - upper - ANSWER94 forceps - upper right molars
(UR6,7,8)
95 forceps - upper left molars (UL6,7,8)
,76 (or 76n) forceps - upper premolars (UR/L4,5)
--> can get right handed AND left handed versions of these
29 (or 29s) forceps - upper incisors and canines (UR/L1,2,3)
Forceps for extractions - lower - ANSWER73 forceps - lower molars
(LR/L6,7,8)
74 (or 74n) forceps - lower premolars and canines (LR/L3,4,5)
74n forceps - lower incisors (LR/L1,2)
Patient position for extractions - ANSWERUpper - head between elbow and
shoulder (move chair to highest it goes, patient at 60% back to upright
Lowers - head at elbow height, patient sat straight up
Operator position for extractions - ANSWERStood in front right for upper
right, upper left and lower left
Stood behind right for lower right
Supporting hand positioning for extractions - ANSWERUpper right and upper
left - finger and thumb protecting alveolus, other fingers outside mouth
Lower left - first finger buccal and second finger lingual and thumb under jaw
to support mandible
Lower right (stood behind patient) - arm comes round patient's left shoulder,
thumb lingual and first finger buccal protecting alveolus, other fingers below
jaw supporting mandible
Movement of 94/95 forceps for extractions - ANSWERFor upper molars
Steady buccal movements - buccal, back to midline
, Until tooth gets to 45 degrees
Repeat until tooth is mobile
Then deliver buccally
NO PALATAL MOVEMENT
Movement of 76 (or 76n) forceps for extractions - ANSWERFor upper
premolars
Steady buccal movements
Until tooth gets to 45 degrees
When mobile, shimmy to lift tooth out
Movement of 29 (or 29s) forceps for extractions - ANSWERFor upper
incisors/canines
Buccal movements then a slight bit of rotation to take the tooth out
Movement of 73 forceps for extractions - ANSWERFor lower molars
Steady buccal movements until tooth is at 45 degrees
Then figure of 8 movement (lingual and buccal) and deliver buccally
Movement of 74 (or 74n) forceps for extractions - ANSWERFor lower
premolars/canines
Rotate until mobile
Then deliver buccally using steady buccal movement
