RN’s are likely to complete nutrition screening, implement nutrition therapy as prescribed by the dietician, complete nutrition related teachings, need to know when to consult a dietician, and RN’s are the median between the dietician & physician. Nutrition Screening - Is a quick look at a few variables to judge a clients relative risk for nutritional problems. It is custom designed for particular populations (e.g pregnant women) or for specific disorders (e.g heart disease). This process is done by RN & it is mandated by JCAHO to be done within 24 hours of patients admission. In acute care settings malnutrition may be illness related & in community settings malnutrition may be psycho-social related. Nutrition Assessment - Clients considered to be at moderate to high risk for malnutrition through screening are usually referred to a dietician for a comprehensive nutritional assessment to identify specific risks or confirm the ex-istence of malnutrition. The steps for this assessment include… - Assessment: Medical history & patient diagnosis, BMI, weight change, dietary intake, physical findings & clini-cal lab values. - Diagnosis: e.g Altered Nutrition - eating less than the body needs may be your diagnosis - Intervention: MNT is the intervention - Monitoring & Evaluation: Observe/document intake, monitor compliance with a restrictive diet, access under-standing of teaching.
