S (Subjective):
Reason for Visit: 6 month exam, Bordetella and ProHeart6 injection.
Owner concerns: patient occasionally will eat her food and then
immediately vomit (in the evening). It happens a few times a week. Patient
is normal otherwise. Changes in the house: Owner is pregnant.
O (Objective):
Temperature: 101.4 degrees – within normal limits (100-102.5 degrees is
normal).
Heartrate: 104 BPM – within normal limits (70-180 BPM is normal for
canines) – No arrhythmias or murmurs auscultated.
Respiratory Rate: 32 RPM – within normal limits; patient taking good, deep
breaths.
Lung sounds – no crackles or wheezes detected.
Pulse: 100 pulse rate – pulse quality is strong.
CRT: < 1 second, gums are moist and pink.
Skin turgor/ hydration status: <5% – indicating patient is not showing any
clinical signs of dehydration.
A (Assessment): Performed by DVM
Patient has no recent foreign body history, abdomen tender with palpation –
unable to palpate deeply. Body Condition Score (BCS) 5/9.
Keeping patient for Barium series – every hour; 3 view abdominal rads.
Assessment of Barium series – barium is moving, no obvious foreign body.
Rads submitted to VetRad for confirmation.
Possible motility issue, developing food incontinence, or partial gastric
obstruction.
DVM will call owner tomorrow with VetRad report.
