Begins immediately after birth with contraction of smooth muscles. Fundus descends 1-2 cm

Lochia

Post birth uterine discharge that lasts 4-8 weeks.

Lochia Rubra

Red color, consists of blood & trophoblastic debris. Lasts 1-2 days post partum.

Lochia Serosa

Pink to brown color. Consists of old blood, serum, leukocytes, & tissue debris. Occurs day 3-5
post partum.

, Lochia Alba

Yellow to white color. Consists of leukocytes, epithelial cells, mucus. Occurs day 10-14 post
partum.

Cervical changes post partum

OS dilated to 10 cm during labor, closes gradually. Within 2-3 days post partum, cervix becomes
firm, regains form, & is approx 2-3 cm dilated.

Cervical dilation by 1 week post partum

Approx 1 cm dilated.

External OS appearance post partum

Never regains pre pregnant appearance, looks like a jagged slit (fish mouth appearance).

Vagina & perineum changes

Greatly distended during vaginal birth, gradually decreases in size & increases in tone. Rugae
reappear within 3 weeks as ovarian function & Estrogen production returns.

Episiotomy

A small surgical incision in the perineum to increase the size of the birth passage during a
vaginal birth.

Laceration

A natural tear in the perineum as a result of cephalopelvic disproportion during a vaginal birth.

1st degree laceration

Involves perineal skin & vaginal mucosa.

2nd degree laceration

Involves the fascia & muscles of the perineal body.

3rd degree laceration

Extends to include the anal sphincter.

4th degree laceration

Extends to include the rectal mucosa.
