Profile of the Client

Brian is a 76-year-old who resigned from his old career as a nurse years before his

actual retirement. Brian was born in South Wales, where he attended St. Michael's School in

Llanelli and, later, the Aberystwyth University. Brian has a long history of hypertension and

acute trauma, which he has been battling for several years now, despite accessing diuretic

therapy. His very first hypertension encounter was after the death of his newly married wife

and son in a fatal road accident. The medical practitioner in charge of Brian's first

hypertension encounter described his condition as a health defect developed after the tragic

road accident that left his whole family deceased. Since then, Brian has been living all by

himself in a small apartment and never remarried again. In 2017, Brian started facing troubles

, in memorising things that were never a problem in the first place. Initially, Brian experienced

infrequent memory relapse, occasionally forgetting names and places, to even not having a

full-on conversation via telephone whenever his general practitioner called to check on his

health progress. Consequently, he was referred by his GP to secondary services for an

assessment, and within four months, he was diagnosed with Alzheimer's disease and

depression.

The community mental health team supports Brian and continues to take his

medication as prescribed to ease the repercussions of Alzheimer's disease and depression. In

addition, Brian relocated from his initial residency in Lavernock to Cardiff, where he

currently resides at Sunrise of Cardiff Care Home. Brian explains that seeking assistance

from senior residence and his community mental health nurse was the best decision he made,

especially now that it was not easy to take care of himself. In one of the most recent visits,

Brian narrated his difficulty to complete familiar tasks due to a troubling understanding of

various visual images or special relationships. Furthermore, he mentioned suicidal thoughts

resulting from compunctions about how healthy he lived before the onset of all these

conditions. Therefore, it is important for Brian that he requires further medical assessments

by using an appropriate assessment tool to identify suicidal ideation. Furthermore, at the

same time, a detailed treatment care plan has been produced. This is important for him as it

helps to reduce every encounter of varied risks, symptoms of his diagnosis and helps him to

counter the worse off health outcomes.

A further medical assessment is crucial as it would help the medical practitioners to

be able to tackle not only Alzheimer's disease but also the suicidal thoughts associated with

depression. Such efforts are priceless as they are the only hope towards identifying and

managing unforeseen medical tragedies related to Brian's medical condition. Nevertheless,

Brian receives massive support from Sunrise of Cardiff Care Home staff and his Community
