APEX NIH Stroke Scale – Group B
Patients 1–6 Assessment Summary
Purpose: To document and evaluate neurologic deficits in acute stroke patients using
the NIH Stroke Scale (NIHSS).
Scoring Key:
0 = Normal / No deficit
1–4 = Minor deficit
5–15 = Moderate deficit
16–20 = Moderate to severe deficit
21–42 = Severe deficit

1a: 0 – Alert
1b: 1 – Answers one question correctly
1c: 0 – Performs both tasks correctly
2: 0 – Normal
3: 0 – No visual loss
4: 1 – Minor paralysis
5a (Left Arm): 0 – No drift
5b (Right Arm): 4 – No movement
6a (Left Leg): 0 – No drift
6b (Right Leg): 3 – No effort against gravity; limb falls
7: 0 – No ataxia
8: 1 – Mild to moderate sensory loss
9: 1 – Mild to moderate aphasia
