History of Present Illness (HPI)
The patient is a 65-year-old male who presents with
progressive fatigue for approximately 6 months. He reports
feeling “tired all the time,” with decreased energy and
reduced exercise tolerance. Fatigue is present throughout the
day and is not relieved by rest. He denies fever, night sweats,
unintentional weight loss, chest pain, or shortness of breath.
