• Setting: Outpatient clinic with laboratory, X-ray, and ECG
capabilities
• Chief Complaint: Loose stools for several days
Reason for Encounter (RFE)
The patient presents with loose stools occurring multiple times per day,
associated with mild abdominal discomfort.
History of Present Illness (HPI)
The patient is a 25-year-old adult who reports experiencing loose,
watery stools for approximately 3–4 days. The stools occur 4–6 times
daily and are not associated with blood or mucus. The patient describes
mild crampy abdominal pain, primarily in the lower abdomen,
relieved after bowel movements.
The patient denies:
• Fever or chills
• Vomiting
