History of Present Illness (HPI)

The patient is a 22-month-old toddler brought in by the mother due to concerns about difficulty
making friends and limited interaction with other children. The mother reports that over the past
4–6 months, the child shows minimal interest in playing with peers during daycare and family
gatherings. The child tends to play alone, avoids group activities, and becomes distressed when
approached by unfamiliar children.

The child maintains good eye contact with caregivers at home but inconsistently responds to
name when called in busy environments. Speech development includes a limited vocabulary of
approximately 10–15 words, with occasional use of gestures to communicate needs. No
regression of previously acquired skills is reported.

There is no history of aggression, excessive tantrums, or self-injurious behavior. The child
demonstrates appropriate attachment to parents and seeks comfort when upset. Appetite, sleep
patterns, and bowel and bladder habits are reported as normal. No recent illnesses, fever, head
trauma, or hospitalizations are noted.
