History of Present Illness (HPI):

, Samantha Graves is an 18-month-old female who presents with vomiting and diarrhea that
began approximately 24 hours ago. According to her mother, the patient has had 4 episodes of
non-bloody, non-bilious vomiting and 6 episodes of loose, watery stools since symptom onset.

The mother reports that Samantha has had decreased oral intake and has been more irritable than
usual. She has tolerated small sips of fluids but vomits when larger volumes are given. No blood
or mucus noted in the stool. No projectile vomiting.

The mother reports decreased urine output, with only one wet diaper in the past 8 hours,
compared to her usual pattern. She denies fever at home but states the child “felt warm.” No
recent travel. No known exposure to contaminated food, but the child attends daycare where
several children have recently been ill with similar symptoms.
