History of Present Illness (HPI):

, Buddy Theodore is a 5-year-old male who presents with increased coughing, wheezing, and
shortness of breath that began 2 days ago and worsened today. His mother reports symptoms
started after a recent upper respiratory infection with nasal congestion and runny nose. Cough
is worse at night and with activity.

He has required his rescue inhaler more frequently with minimal relief. No chest pain reported.
No cyanosis. Appetite is slightly decreased but fluid intake is adequate. No vomiting or diarrhea.

No recent hospitalizations but has had prior asthma exacerbations treated with albuterol.

Associated Symptoms:

• Wheezing
• Shortness of breath
• Chest tightness
• Nighttime cough

• Fever
• Vomiting
• Diarrhea
• Rash
• Lethargy
