SUBJECTIVE
CC: “I’m peeing all the time, and it burns. Google says I should see my doctor.”
HPI: 46-year-old male with PMHx of obesity, who presents with increased urinary
frequency, dysuria, and suprapubic discomfort that began a week and a half ago.
Accompanied by right flank pain that started yesterday. Rates the right flank pain as 4/10
in intensity, described as a dull ache, without radiation. He has not taken any medications
for symptom relief. Denies hematuria, penile discharge, fever, chills, nausea, vomiting,
testicular pain, or concern for sexually transmitted infections. No prior history of UTIs or
kidney stones.
PMH: Obesity
PSH: None
Family History: Father: HTN and ERSD on HD, alive, age 71
Social History:
The patient is married. Sexually active with 1 female partner (wife), no new partners.
Denies condom use. Works from home and reports adequate hydration and bathroom
breaks. Reports occasional alcohol use but denies tobacco use and illicit drugs. No recent
traveling.
Medications:
None
Allergies:
No known drug allergies.
