Donald, a 60-year-old male with dyspnea, chronic productive cough, wheezing and
fatigue. History of smoking 1 pack/day for 40 years. Post-bronchodilator FEV1/FVC ratio < 0.70,
indicating persistent airflow limitation (likely COPD). Donald’s symptoms and smoking history
suggest Chronic Obstructive Pulmonary Disease (COPD). The goal of treatment is to reduce
symptoms and prevent flare-ups. Smoking cessation is vital to halt disease progression.
Treatment Plan:
For Donald's treatment plan, I would incorporate smoking cessation support, including a
gradual tapering strategy to reduce his dependency on cigarettes. To address the psychological
aspects of quitting, I would provide behavioral support (Onwuzo et al., 2024). Complementing
this, I would initiate bronchodilator therapy to alleviate his dyspnea and improve lung function
(Agustí et al., 2023). A long-acting bronchodilator will help open his airways, reduce symptoms
like wheezing, and improve his overall well-being. This comprehensive approach aims to
effectively support Donald in managing his COPD while addressing his smoking habits.
Medication Prescription:
For Donald's medication prescriptions, I would include Varenicline to support his
smoking cessation efforts. This would help reduce cravings and withdrawal symptoms.
Additionally, I would prescribe Tiotropium at a dosage of 18 mcg, inhaled once daily, to improve
his lung function and alleviate symptoms of COPD. This combination of medications will provide
him with the support he needs to quit smoking while effectively managing his respiratory
condition.
