, What is the Supports Intensity Scale (SIS)? - Answer: The SIS is a multidimensional scale
designed to determine the pattern and intensity of an individual's support needs. The SIS is to
be used with all DDD clients age 16 and older. The use of the SIS by the Division of
Developmental Disabilities is by license from American Association on Intellectual and
Developmental Disabilities (AAIDD).
How is the Intermediate Care Facility for the Mentally Retarded (ICF/MR) Level of Care
determined? - Answer: The Intermediate Care Facility for the Mentally Retarded (ICF/MR) Level
of Care for persons age 16 and older is calculated by an algorithm using assessment scores from
the SIS.
The SIS should be completed without regard to the services or supports currently provided,
available or desired. - Answer: The assessor and the respondent(s) should not consider support
services or natural supports that are currently being provided. Instead, each activity should be
rated according to what dimensions of support (i.e. Type of Support, Frequency and Daily
Support Time) are needed to best meet the needs of the person being assessed.

Frequency: - Answer: Select the frequency which most closely describes how often
"extraordinary support" (i.e. support beyond that which is typically needed by most individuals
without disabilities) is required for each targeted activity. Frequency is rated on a 0-4 scale, with
higher numbers indicating greater support needs.
