The document is a detailed Biopsychosocial Assessment report for a 13-year-old male, E.R., an African American middle school student. The assessment aims to understand the root causes and develop an intervention plan for his presenting problem of emotional control issues, specifically temper and irritability. The report systematically documents E.R.'s background, current problems, and history using a biopsychosocial framework. Presenting Problem & History: Problem: Emotional control issues, characterized by irritability and temper management problems, which have been slowly deteriorating over the last six months. Referral Source: School counselor. Impact: The condition has adversely affected his academic, social, and emotional wellbeing , and causes distress to his family. Past Intervention: Enrollment in an Individualized Education Program (IEP) and compulsory school counseling sessions. Trauma/Stressors: Key stressors include parental separation and subsequent relocation to a new, poorer neighborhood, a new school, financial struggles for his mother, and teasing at school. E.R. blames his mother for the breakup. Formulation (Systematic Factors): Psychological: E.R. demonstrates articulate self-awareness and strong cognitive ability but is disturbed by the reality of his parents' separation and feels powerless. Symptoms like withdrawal, defensiveness, and unwillingness to express himself point to emerging traumatic stress. Environmental/Social: Distressed by the emotional distance created by the separation and struggles to adjust to the new environment and school. He lives in a poor neighborhood. Cultural/Diversity: Being African American is noted as a risk factor for trauma due to systemic issues like racism and biased access to quality mental health. Strengths: E.R. is sociable, self-aware, open, and positively motivated to overcome his problems , making him likely to comply with treatment. He has a strong support system from family (parents, grandparents) and school (teacher, counselor)
