✔✔One year - ✔✔365 consecutive days from the first day the individual teaches in Ohio

✔✔Pursuant to the the rules described above in regards to teaching PT without an Ohio
license the PT must notify the PT section with a notification that should include - ✔✔1)
the individuals name

3) the name and number of the course(s) that the individual will teach
4) the date that the individual will commence teaching in ohio
5) the license number for all valid PT licenses held in the US or any foreign country

✔✔The OTPTAT shall grant a PT License by Reciprocity (an agreement between states
to issue licenses to applicants from other states with credit for their courses or license
level to reduce requirement) if all of the following are true - ✔✔1) Passed NPTE
2) Passed Jurisprudence
3) Holds a current and valid license or registration to practice PT in another state or
country
4) Minimal education requirements met
5) the applicant pays the fee described in section B of 4755.42 of the revised code
6) the applicant is not in violation of any section of this chapter or rule adopted under it

✔✔If the applicant's education is not reasonably equivalent to the educational
requirements that were in force for licensure in the state on the date of the applicant's
licensure... - ✔✔the section shall send a written notice to the applicant denying the
applicant's request and state the specific reason why. Sending this notice through
certified mail within 30 days after the section makes that determination.

✔✔If a non-ohio licensed PT plans on teaching for more than one year... - ✔✔the
individual must obtain an Ohio PT license prior to the completion of the one year period

✔✔To renew your PT license... - ✔✔Renewal fee is not more than $125, the applicant's
signed statement that the applicant completed the CEU requirements within the
timeframe established

✔✔A license that is not renewed - ✔✔will automatically expire on its expiration date

✔✔Loss/suspension of licensure can occur from... (1-10) - ✔✔1) habitual indulgence in
the use of controlled substances
2) conviction of a felony or a crime
3) Obtaining or attempting to obtain a licensed issued by the PT section by fraud or
deception
4) An ajudication by a court that the applicant or licensee is incompetent for the purpose
of holding the license

, 5) violation of the code of ethics
7) violating/attempting to violate sections 4755.40 to 4755.56 or any issued rule adopted
by the section(s)
8) failure of one or both of the examinations required for licensure
9)Permitting the use of one's name or license by a person, group, or corporation when
the one permitting the use is not directing the treatment given
10) failure to maintain minimal standards of practice in the administration or handling of
drugs, or failure to employ scientific methods in the selection of drugs or other
modalities for treatment

✔✔Loss/suspension of licensure can occur from... (11-20) - ✔✔11) willful betrayal of
professional confidence
12) making a false, fraudulent, deceptive or misleading statement in the solicitation of or
advertising for patients in relation to the practice of PT
13) a departure from, or the failure to conform to minimal standards of care required of
licensees when under the same or similar circumstances, whether or not actual injury to
a patient is established
14) obtaining, or attempting to obtain money or anything of value by fraudulent
misrepresentations in the course of practice
15) violation of the conditions of limitation or agreements placed by the PT section on a
license to practice
16) failure to renew a license
17) engaging in the division of fees for referral of patients or receiving anything of value
in return for a specific referral of a patient to utilize a particular service or business
18) inability to practice according to acceptable and prevailing standards of care b/c of
mental illness, physical illness, including physical deterioration that adversely affects
cognitive, motor, or perception skills
19) the revocation, suspension, restriction or termination of clinical priviledges by the
US dept of defense, VA
20) Termination or suspension from participation in MEdicare or medicaid program
established under Title XVIII and XIX of the Social Security Act

✔✔Loss/suspension of licensure can occur from... (21-28) - ✔✔21) failure of a PT to
maintain supervision of a student, PTA, unlicensed support personnel, other assistant
personnel, or a license applicant
22) failure to complete CEU requirements
23)conviction of a misdemeanor when the act that constitutes the misdemeanor
occuring during the practice of PT
24) failure to cooperate with an investigation conducted by the PT section, including
failure to comply with a subpoena or orders issued by the section or failure to answer
truthfully a questions presented by the section at a deposition or in written
interrogatories
25) regardless of whether the contact or verbal behavior is consensual, engaging with a
patient other than the spouse of the PT or PTA in sexual contact or verbal behavior
26) failure to notify the PT section of a change in name, business address or home
address within 30 days of the change
