NUR 6001 SHADOW HEALTH: GI | Fall 2025/26 Update with complete solutions. Tina Jones: 28 year old African American woman. Pt A&O x3 and reliable; able to provide her health history and CC. C/o epigastric stomach pain that started one month ago and is getting worse over time; now every time she eats within 10-15 minutes after a meal with increased belching. Denied N/V, wgt loss, fatigue, fever, chills, change in color of stool or recent ABD trauma. Pain localized between bellybutton & ribs, denied radiating pain. Pain lasts a few hours after she eats and feels worse when she bends over and lays down. Feelings of heartburn, sharper at times, pain scale 5/10. Denied crushing, gnawing, or tender to touch. Currently pain 1 or 2 out of 10. Diet is a contributing factor, ie: fast food, foods high in acidity, confirmed pizza yesterday and orange juice this morning. Denied eating spicy food & decreased meal size. Last alcoholic drink, two wks ago, with irritation to stomach. Increase in water intake, making urine light-yellow to clear, denied pain or blood tinge. Reported nocturia. Denied swallowing difficulties. Increase stressors, finishing school, job hunting, and currently working. Stomach pain is relieved with antacids, break in-between meals, and sitting up. Stated due to pain she doesn’t want to eat.
