What does a modifier do? - ANSWER-Makes a basic or important change in a
CPT code
•Used when the CPT code alone does not accurately describe the entire picture
•Allows us to detail what exactly was done for the patient
•Can increase reimbursement
Modifier -22 - ANSWER-Increased Procedural Services
●Services rendered took more time, work or was unusual from what was expected
●Used for procedure codes - not used with E/M codes
●Need to provide documentation and usually a cover letter demonstrating why
service is unusual.
Example: During surgery, a patient lost 600 cc. of blood requiring intraoperative
transfusion.

Modifer -23 - ANSWER-Unusual anesthesia

of general anesthesia
●Medical record must explain the need for general anesthesia
Example: A mentally challenged patient is extremely anxious and apprehensive

Modifer- 24 - ANSWER-Unrelated E/M Service by Same Physician During
Postoperative Period
●Used only with E/M codes
●Used during global surgery follow-up period to indicate E/M service provided
during global surgical period but not related to surgery performed.
Example: One week after surgery to release a frozen shoulder, patient was seen for
sprained ankle Report code 99213-24.
Modifer -25 - ANSWER-Significant, Separately Identifiable E/M Service by the
Same Physician on the Same Day of the Procedure/Service
●Used with E/M codes
●Used to indicate a procedure or service was provided that was above and beyond
what is normally associated with the procedure or service. (More info in notes)
