Painful Rash - (Class 6560)
, History of Present Illness (HPI)
Mr. X is a 60-year-old male who presents with a painful rash on the
right side of his chest that began 3 days ago. He first noticed tingling
and burning discomfort in the area, followed by the appearance of
grouped red bumps that have since progressed to fluid-filled
blisters. The pain is described as burning, constant, and rated 7/10.
He denies trauma or new exposures. No recent changes in
detergents, soaps, or medications. He has not had fever, chills, or
recent illness.
Past Medical History (PMH)
• Hypertension, well-controlled with lisinopril
• Hyperlipidemia
• No history of diabetes
• No known immunosuppressive conditions
Past Surgical History (PSH)
• Appendectomy in his 20s
• No other surgeries
Medications
• Lisinopril 10 mg daily
• Atorvastatin 20 mg daily

Family History
• Father: deceased, MI at age 72
