Impact of Clinical Practice Problem
, Enhancing Patient Safety through Standardized Handoff Tools in Pediatric Inpatient Units Revised
Ineffective communication during patient handoffs in acute pediatric inpatient units
significantly impacts patient safety and the credibility of healthcare organizations. This
communication gap can lead to increased medical errors, which are detrimental to patient
outcomes and can tarnish the reputation of healthcare institutions. To address this issue,
implementing a standardized patient handoff tool is proposed. Such an intervention aims to
enhance the clarity and efficiency of communication, thereby reducing the likelihood of errors
and improving overall patient care quality.
Impact of the Problem on the Organization
Ineffective communication during patient handoffs in a pediatric inpatient hospital
organization can have far-reaching impacts, affecting patient safety, financial stability, legal
standing, staff well-being, reputation, compliance with healthcare standards, and overall
operational efficiency.
• P (Patient, Population, or Problem): Pediatric patients in acute inpatient units experiencing
ineffective communication during handoffs.
• I (Intervention): Implementation of a standardized patient handoff tool.
• C (Comparison): Current practice of unstandardized patient handoff processes.
• O (Outcome): Reduction in medical errors and improvement in patient safety and
communication effectiveness.
Evidence Based Practice Question: In pediatric patients in acute inpatient units (P), how does
the implementation of a standardized patient handoff tool (I), compared to the current practice of
