AFT_2_TASK_2 Accreditation Audit (AFT2) Task 2 C A1. EVENT DESCRIPTION THIS IS A REVIEW OF A SENTINEL EVENT THAT OCCURRED AT NIGHTINGALE AMBULATORY SURGERY UNIT. A 3-YEAR-OLD FEMALE WAS BROUGHT IN BY HER MOTHER FOR SURGERY. UPON ARRIVAL, THE CHILD WAS REGISTERED, AND ALL CONSENTS WERE OBTAINED FROM THE CHILD’S MOTHER. NURSING PERFORMED PRE-OP DUTIES AND WHEN THE CHILD WAS HANDED OFF TO THE CIRCULATING NURSE FOR SURGERY THE MOTHER INFORMED THE PRE-OP NURSE THAT SHE WOULD BE BACK TO GET THE CHILD WHEN THE SURGERY WAS COMPLETE AND LEFT HER CONTACT INFORMATION IN THE SURGERY WAS DONE EARLY. Sentinel Event (AFT2) Task 2 A1. Event Description This is a review of a sentinel event that occurred at Nightingale Ambulatory Surgery Unit. A 3-year-old female was brought in by her mother for surgery. Upon arrival, the child was registered, and all consents were obtained from the child’s mother. Nursing performed pre-op duties and when the child was handed off to the circulating nurse for surgery the mother informed the pre-op nurse that she would be back to get the child when the surgery was complete and left her contact information in the surgery was done early. The pre-op nurse did not hand off the mother's contact information or the situation of the mother coming back to pick the child up was not communicated. The surgery went as expected and was completed early and the child was transitioned into PACU where that nurse attempted to page the mother overhead in the waiting room, but she never responded. After being fully recovered, the child was handed off to the discharge nurse.
