When these individuals are admitted, A care plan for them must be done in order to ensure
that support and everything for their care are provided. This need for a care plan ensues
health care professionals to undergo the process of a care planning cycle these are:
1. Referral - the first step in the process for providing a support for the individuals
would be from referrals. A referral as defined by www.who.int is “a process in which
a health worker at a one level of the health system, having insufficient resources
(drugs, equipment, skills) to manage a clinical condition, seeks the assistance of a
better or differently resourced facility at the same or higher level to assist in, or take
over the management of, the client’s case.”
