When an individual needs support from health care professionals, they are provided with a
plan of support in order to ensure all of their needs are met. This plan is made using different
assessment tools and processes. In this coursework, I will be Identifying these processes
and assessment tools and try to relate it all in the end using the case studies provided for us:

Case studies:

1. Mr.Farsal has just returned from a trip to see his brother in Egypt.
Unfortunately Mr. Farsal was involved in a car accident on the way from the
airport and was badly injured. Mr. Farsal has been told by his doctor that he
will not be able to use his arms and legs until they heal in about 2 weeks.

2. Maureen has sight impairment and suffers from multiple sclerosis. Maureen is
now unable to meet her own personal care needs. Use the appropriate sections
of your workbook to describe the different personal care needs and emotional
care needs that have to be met by a carer on Maureen’s behalf.

When these individuals are admitted, A care plan for them must be done in order to ensure
that support and everything for their care are provided. This need for a care plan ensues
health care professionals to undergo the process of a care planning cycle these are:

1. Referral - the first step in the process for providing a support for the individuals
would be from referrals. A referral as defined by www.who.int is “a process in which
a health worker at a one level of the health system, having insufficient resources
(drugs, equipment, skills) to manage a clinical condition, seeks the assistance of a
better or differently resourced facility at the same or higher level to assist in, or take
over the management of, the client’s case.”

Although at the same time, referrals could also be done by the individual
themselves e.g. going to see the specialist by themselves because of concerns they
want to tackle immediately with an expert, a third party like for example when a
