Mixed dysarthria

Heterogeneous group

Occurs when neurologic damage extends into two or more parts of motor system

Characterized by combination of characteristics in single (pure) dysarthria

Neurologic damage crossing anatomical boundaries

Mixed dysarthria: Neurologic Basis

Neurologic damage crossing anatomical boundaries

Any combination of pure dysarthrias

Prominence of each pure dysarthria within mixed dysarthria varies significantly among
individuals

Mixed dysarthria: Etiologies

Degenerative (motor neuron disease, ALS, Friedrich's Ataxia, multiple system atrophy)

Chronic traumatic encephaolopathy

Vascular disorders (Strokes)

Toxic-Metabolic conditions (WIlson's disease)

Demyelinating disordes (Multiple Sclerosis)

,TBI

Motor Neuron Disease (MND)

Collective deficit; ALS

Amyotrophic Lateral Sclerosis (ALS)

Progressive degeneration of any of four areas of motor neurons

Often progresses from affecting two motor neuron groups to all four

-Spinal nerves where they join the spinal cord = flaccid

-Cranial nerves where they join the brainstem = speech

-UMN: corticobulbar & corticospinal tract = spastic

UMN & LMN signs in the limbs or bulbar muscles or both

Diagnosis: clinical profile & testing

Type of mixed dysarthria: mixed flaccid-spastic

, Clinical features: Fatigue, Cramping, Fasciculations, Muscle atrophy, Spasticity, Cognitive deficits

Speech: Imprecise consonants, Hypernasality, Harshness, Slow rate, Monopitch, Short phrases,
Distorted vowels, Excess & equal stress, Strained-strangled voice, “wet” or “gurgly” voice quality

Differential Diagnosis: Progressive muscular atrophy/Spinal muscle atrophy, Progressive bulbar
palsy, Primary Lateral Sclerosis, PLS

Friedrich's Ataxia

Causes neuron degeneration in cerebellum, brainstem, and spinal cord

Inherited degenerative disease

Mixed dysarthria: mixed ataxic-spastic

Speech: Harshness, Breathiness, strained-stangled voice quality, Audible inspiration, Monopitch,
Imprecise consonants & distorted vowels, Abnormal rate, Excess & equal stress

Multiple systems atrophy

Neurodegenerative condition

Collective term for group of degenerative disorders, many including parkinsonian symptoms
