Why is your patient in the hospital (Answer in your own words and include the History of present Illness): Mrs. Cole, a 31-year-old female, was admitted to the
hospital due to a fracture of her right ankle, which required surgical intervention. The patient suffered a fall, which resulted in a fracture of her ankle, as she had
an altercation with a band member on a local bar stage. The patient has a history of bipolar disorder, which was diagnosed six years ago. The patient’s husband
reports that he and the patient's sister had been out looking for the patient as she had disappeared. The husband states that the patient typically adheres to the
medication regime of Lithium 600mg twice a day; however, he recalls that the patient had not been sleeping or eating well in the last couple of weeks. The
patient’s husband states that there is a history of the patient stopping medications, which last occurred two years ago and resulted in a manic episode.
Health History/Comorbities (that relate to this hospitalization): Right ankle fracture with surgical repair, bipolar disorder.
Shift Goals/ Patient Education Needs:
1. Patient will adhere to medication as prescribed and maintain a therapeutic level of lithium within 24 hours.
