LABEL COMPONENTS
- Trade name = assigned name by manufacturer, the brand name, not used by everyone
- Generic name = official name used by all companies
- Form = composition of a drug (tablet, suspension, capsule)
- Controlled release (CR)
- Sustained release (SR)
- Extended release (XR)
- Total contents = for multidose containers
- Unite dose strength (concentration) strength + form
- EX: 500 mg per tab; 50g per mL
- Route
- Expiration date
- Lot number
- Label alerts
- USP (united states pharmacopeia) Safety standards for medications; and National
Formulary (NF)
- Barcode
Controlled medications:
- Schedule I = highest risk for abuse
- Schedule II through V - prescribed for medical purposes
Tall Man letters for sound-alike, look alike drugs
- Used of mixed-case or enlarged letters to emphasize the differing portions of two drug
names

Medication Error = any preventable event that may cause or lead to inappropriate
medication use or patient harm while the medication is in the control of the healthcare
professional, patient, or consumer.

What contributes to medication errors: failure to follow rights, calculation errors, lack
of assessment, look alike/sound alike, lack of knowledge, wrong transcription,
documentation error, abbreviations, incomplete delivery of meds.
REMEMBER: nurses can be held legally responsible for medications given, even if the
order or dose was in error.
