Onset: Rash began 4 days ago
Location: Broke out on arms and spread to chest and back
Duration: Recalcitrant since onset, refusing to get better
Character: Red, raised, itchy patches; some small blisters
Associated Symptoms: Mild heat, no systemic or fever
Aggravating Factors: Scratching, warm weather, tight clothing
Relieving Factors: Cold showers, over-the-counter hydrocortisone cream (short relief)
