models of care that are adopted by some of the LTCFs. These models promote
meaningful environments with individualized approaches to care.

Culture Change - answer A term given to the process of transforming services for elders
so that they are based on the values and practices of the person receiving care. Culture
change involves respecting both elders and those working with them. Core values are
promoting choice, dignity, respect, self-determination, and purposeful living. To honor
culture change, care settings may need to change their organization practices, physical
environments, and relationships.

Person-directed care - answer Emphasizes the individuality of the person who needs
care, and seeks to build community by recognizing and developing each person's
capabilities. Promoting dignity and providing caring environments is the key.

Medicare - answer A federal health insurance program that was established in 1965 for
