$8.00 Dollar Fee - ANS-fee paid to county agriculture commissioner for any fumigation
performed in county. (La, OC, SCC, SD). Paid by the 10th of the month with list of all
fumigations
Ag department notification - ANS-notify at least 24 hrs prior, mailed or via phone.
Approved Secondary locks - ANS-locks approved are: Clamshell, padlocks, keyway locks, pins,
inside deadbolts.
Authorization of fumigator - ANS-A written statement must be given to consumer before sub
contracting fumigation work to a fumigator.
Branch 1 required training - ANS-A minimum of one hundred hours of training and experience in
preparation, fumigation, ventilation and certifications required.
Chloropicrin - ANS-tear gas
direct and personal supervision - ANS-branch 1 must present on fumigation site entire time
fumigant is released, ventilation is commenced and when released for occupancy
Direct fumigation - ANS-The direct fumigator is when the consumer decided to use the report
and contract the fumigation directly.
Exterior opens (no door) - ANS-Impassable barricades into living space durning fumigation
Fire department notification - ANS-notify no less than 2 hours prior, no fee. Will be fined if
emergency response due to negligence.
Five working days (refumigation) - ANS-Fumigator within five days must notify the date of the
refumigation on a "direct" fumigation. With the consumer name and to the branch 3 company
they used.
Fume Log (record keeping) - ANS-log must be retained for 3 years
Fumigants - ANS-methyl bromide; Sulfuryl fluoride (vikane/profume); Aluminum phosphide
fumigation - ANS-use within enclosed space to destruct plant or animal life, with substance
pressure 5 millimeter of mercury at 25 degree centigrade
Fumigation log (Prime contractor) - ANS-Fumigator has ten days to submit fumigation log to the
prime contractor once the fumigation is completed.

No, Warning agent, type of seal, weather, cylinder no,Date/Time,first&surname of crew, aeration
time,released for re-entry, operator/FR ventilating, clearing device, ...
Fumigation tag (5"x7") - ANS-immediately upon completion post in the attic, adjacent to
entrance.
Fumigator - ANS-fumigator should be able to administer CPR
Garage w/out car door - ANS-Garage without car door will be considered a carport and will not
need barricaded, unless evidence that the garage area is being used for habitation.
info on fumigation tag (5"x7") - ANS-company, date of fume, fumigant
no signed Occupants Fumigation notice - ANS-the prime company along with fumigator will be
fined for not having Occupants Fumigation notice signed properly.
