provider believes that Medicare will probably not pay for services receive
Copayment - a fixed fee paid by the patient at the time of an office visit
Explanation of Benefits (EOB) - describes the services billed and includes a breakdown
of how the payment is determined (sent to pt)
what is CMS-1500? - The CMS-1500 is used by physicians and other non-institutional
providers and suppliers
