As a health and social care worker, we want to provide the best possible care we can to our
patients and through the Person-Centered practice, we can. Person centered practice aims
to help our service user by including them with all the planning, decision-making and
services they may need through a Care plan. This care plan is a part of this approach which
is made together by the service user and the health care worker in both of their agreement
or consent that will assess the needs of the individual and reflect from it so, we can plan
better, implement it and evaluate it accordingly and regularly to identify any problems and
improve the care plan based on its results in the patient’s health. But how does these help
support the individuals involved?


1. It upholds the autonomy and independence of
the patient or service user.

Person centered practice is an approach where it upholds
the autonomy and independence of the individual by
involving them in their care plan to help make choices for
their health. By doing so, It recognises the freedom of the
service user to make a choice, also called autonomy, by
including them in the decision making and letting them
share what they think may help with their health so that it
could be effective as possible

.


2. The Person centered approach is supportive to the patients or service users.

This approach is supportive in a way where the health care
worker acknowledges the choices the patients made that will
help them improve their health. It lets us support them by
helping them make a choice that is proper and effective and
person centered practice also helps the individual feel
encouraged by the support the health and social care worker
is giving. Being encouraged means being motivated to do
something for the better. In this case, it is motivating the
individual to have better health by being helpful to them
through assisting them in the things they need, by being kind
to them through understanding them and by caring for them
properly.




3. It recognises the principles and values of
the patient or service user.
Making a care plan with the service user means that
they will make choices which are based on their
beliefs, ideas and values. Through recognising them,
