Vulnerable Population Concept Map and Summary

Introduction

Population health and epidemiology help discover and treat community health inequities.

This research examines the health risks and inequities of the homeless, a vulnerable group with

many barriers to healthcare and essentials. By evaluating the elements that cause these health

difficulties and linking them with national health directives, we may better comprehend the

problem and devise effective solutions. This paper will describe the homeless population,

analyze their health risks, and highlight their health disparities. Additionally, the report will

outline a Healthy People 2030 national health goal and offer a plan for advanced nursing

practice to advocate for homeless health.

Summary of Concept Map Overview

This paper's concept map covers the health risks and disparities of the homeless, a

vulnerable group that lives in shelters, transitional housing, or on the streets due to a lack of

stable, regular, and adequate nighttime residences. The graphic shows that poverty, poor

healthcare, and unhealthy living conditions put this group at danger.

Lack of secure housing causes several health issues for people experiencing

homelessness. This group includes people of different ages, genders, and backgrounds, but

poverty, unemployment, and social isolation make them vulnerable to health inequities.
