, Interdisciplinary Plan Proposal
The interdisciplinary plan is to increase interdisciplinary collaboration and
communication. This plan will be aimed at the outpatient urology office. The desired outcomes
are to provide quality patient care and facilitate open communication amongst disciplines to
ensure staff feels supported in their role.
Objective
The plan is to reinforce established leadership and refresh staff on roles and
responsibilities their job description entails. Then create a theme-oriented road map with clearly
set expectations and steps necessary to achieve goals. Setting the expectation would require
collaboration from all disciplines to advocate the changes they would like to see. If the process is
successful, the unanswered messages will decrease, patient satisfaction will increase and the
workload will be distributed evenly amongst staff.
Questions and Predictions
1. How long should implantation take? (Pogue, 2021)
a. The projected time frame for execution to be complete would be anywhere from
6-12 months. This time frame was calculated because of the staff habits that need
to be broken and the back work that the office needs to catch up on before the new
workflow can run efficiently.
2. Who will train the staff? Who will be a resource? (Pogue, 2021)
a. Each discipline will have a clinical manager who specializes in interdisciplinary
collaboration. The implementation team leaders will then meet weekly to discuss
progress, issues, or concerns. The results of the meeting will determine whether or
not the roadmap needs to be adjusted.
3. What does the office need before implementation can begin? (Pogue, 2021)
a. The office needs Epic software tailored to the outpatient setting. This software is
for inpatient hospital use and presents challenges. The staff will also need to be
trained to utilize the updated epic software.

The Change Theory written by Reinholz and Andrews focuses on how to implement
change. The literature consists of methods used to identify preconditions that need to be
