Trapezius- Upper fibers (Scapular elevation) - ✔✔Patient- Short sitting with the hands
relaxed in lap
Examiner- Standing behind the patient hands on top of both shoulders to give resistance
in a downward direction. Instruct position of patient to shrug their shoulders or raise
their shoulders toward their ears and hold it.



Trapezius- Middle Fibers (Scapular Adduction-Retraction) - ✔✔Patient- prone with
shoulder at edge of table, shoulder abducted to 90* and externally rotated.
Examiner- PLace one hand on the opposite scapula to prevent trunk rotation and the
other on the test forearm to provide resistance in a downward direction. Instruct the
patient to raise arm off table and hold it.



Trapezius- Lower Fibers (Scapular depression- adduction) - ✔✔Patient- Prone with
arm abducted to about 145*
Examiner- Place one hand below the scapular and the other on the test distal humerus
to provide resistance in a downward direction. Instruct the patient to lift their arm
toward the ceiling and hold it.



Shoulder extension - ✔✔Patient- Prone with the arm adducted extended and
internally rotated.
Examiner- standing at test side, stabilize upper body with one hand and place other
hand on distal humerus and provide pressure in the direction of arm flexion.
Muscles tested- Latissimus dorsi, teres major, posterior deltoid.

, Shoulder flexion - ✔✔Patient- Short sitting with shoulder flexed at 90*.
Examiner- Standing at test side with one hand stabilizing the shoulder and the other on
the distal humerus providing resistance downward as the patient attempts to flex the
shoulder.
Muscles tested- Anterior deltoid, supraspinatus, coracobrachialis, long head of biceps
brachii, clavicular fibers of pectoralis major.



Middle deltoid (Shoulder abduction) - ✔✔Patient- Short sitting with the arm
abducted to 90*
Examiner- Standing behind patient with one hand placed proximal to the elbow. Instruct
the patient to abduct the arm and hold it as you apply downward resistance



Posterior deltoid (Shoulder horizontal abduction) - ✔✔Patient- Prone with the
shoulder abducted to 90* and forearm off edge of table with the elbow flexed.
Examiner- Standing at test side, place one hand proximal to the elbow to provide
downward resistance. Instruct the patient to lift their elbow toward the ceiling and hold
it.



Cervical Extension - ✔✔Patient- Prone with the head off the end of table and the
arms at the side
Examiner- Standing beside the patients head with one hand behind the head to provide
resistance and the other hand beneath the chin, instruct the patient to push against
your hand and hold it while still looking at the floor.
Pressure- Applied to the posterior aspect of head pushing towards the ground
Muscles tested- Splenius capitus/cervicis, Eractor Spinae



Cervical flexion - ✔✔Patient- Supine with the head supported on the table and arms
at side
