ITLS
ITLS patient assessment - ✔✔Scene sizeup, --> initial assessment --> mechanism of injury? -->
generalized or unknown MOI= rapid trauma OR localized MOI= focused exam --> load and go? ---> ITLS
secondary survey OR itls ongoing exam
initial assessment - ✔✔general impression, LOC, ABC
"fix it" process - ✔✔Used because experience shows that most mistakes occu because the tram leaser
stops to perform an intervention and forgets to perform part of the assessment. To prevent this, when
an immediate intervention is needed, delegate it to your team members while you continue the
assessment
Adult normal repiratory rate - ✔✔10-20
adult abnormal respiratory rate - ✔✔<8 and >24
normal small child respiratory rate - ✔✔15-30
abnormal small child respiratory rate - ✔✔<15 and >35
normal infant respiratory rate - ✔✔25-50
abnormal infant respiratopry rate - ✔✔<25 and >60
Ongoing exam - ✔✔each time the patient is moved
each time an intervention if performed
any time the patient's condition worsens
Ask the patient how they feel
LOC
ABCs
, Abdomen
check each indentified injury
check interventions
Secondary survey - ✔✔Meant to pick up additonal injuries
repeat initial assessment
consider using a monitor
record vital signs
do a brief neuro exam- LOC, GCS, pupils, motor skills, sensation
flail chest - ✔✔occurs with the fx in two or more places causing instability of the chest wall and
paradoxical movement on the "flail segment". needs PEEP.
purpose of the primary survey - ✔✔to determine if immediately life-threatening conditions exist and to
identify those patients who should have immediate transport to the hospital. Should happen in less than
two minutes
when to stop your assessment - ✔✔scene becomes unsafe
cardiac arrest
airway obstruction
on scene goal time for critical patients - ✔✔five minutes
If cannot speak or is unconscious, further evaluation of airway should follow - ✔✔Look, listen, and feel
for movement of air.
GCS and when - ✔✔To be done at the end of every rapid trauma survey
Eye opening Verbal motor
Spontaneous...4 Oriented...5 obeys commands..6
