Three types of corticosteroids - Answer 1. Androgens
2. Glucocorticoids
3. Mineralocoticoids
Long term use of glucocorticoids and mineralocticoids can cause: - Answer - body
wasting, adrenal atrophy
- * need to be weaned off
- can interfere with the negative feedback system
Corticosteroids causes what kind of response - Answer 1. HTN (increased blood
volume)
2. Release glucose for energy (may cause increase blood sugar)
3. Decreased protein function (conserves energy)
4. "good thing" Blocks the activity of the inflammatory response (can effect locally,
systemic, topically) (conserves energy for fight or flight)
5. Tamp down the immune response to any any part of the body.
What are some reasons to give corticosteroids long term? - Answer 1. COPD
2. Chronic auto-immune disorders
3. Chronic bronchitis
4. Active airway disorders
Watch for these symptoms when taken long term - Answer - edema
- peptic ulcer disease
- delayed wound healing
- infections
- osteoprosis
Glucocorticoids - Answer - stimulates increase glucose levels (fight or flight)
- causes storage of body fat
, - block the immune response
- contraindicated in presence of acute infection so you must treat the underline cause
- don't give during lactation (can mess w/babies feedback reception)
- precaution in diabetic patient (high sugar), ulcers (bleeding), and other endocrine
disorders
Mineralocorticoids (aldosteron) - Answer - Check electrolyte levels
- not given a lot
- indicated for replacement therapy in adrenal insufficiency and treatment of
salt-wasting problems.
- causes water and salt retention (don't give to HTN, HF, Renal Failure diseases so as to
not increase volume)
- increases potassium excretion
- can cause hypokalemia (s/s: flattened T waves, muscle cramps, hypo-active reflexes)
- Usually taken with glucocorticoids (decadron) not by self
Thyroid Agents - Answer - By way of T3 and T4 produces thyroid hormone
- take 1st thing in morning on empty stomach with a full glass of water (or it will not
absorb well)
- in morning so as not to keep you up all night
- after you give you might see flushing, elevated HR
- can be given to pregnant women (need might be decreased)
- most common used is synthroid (used for hormone replacement therapy in hypothyroid
states; which can be from non-functioning thyroids or the removal of a thyroid
Thyroid regulates: - Answer - metabolic rate
- heat production
- body temp
- growth and development
Hypothyroidism causes: - Answer - weight gain
- low body temp (always cold)
- sleepy/tired
