Description of Procedure
Following overnight fasting, avoidance of caffeine, and abstinence from smoking for 12 hr prior to testing; a
fasting glucose is obtained, a 100g glucose load is given, and serum glucose levels are determined at 1, 2,
and 3 hr following glucose ingestion
Indications CONSIDERATIONS
When 1 hr glucose tolerance test is positive Nursing Interventions (pre, intra, post)
with BS level reading 130-140 mg/dL or
greater, OGTT will be indicated -Obtain blood sugar levels after 1, 2, and 3
hours after loading dose is given.
Interpretation of Findings
Client Education
- A dx of GDM can be made only after an -Educate the client about the need to fast 12 hr prior to the test
-Avoid caffeine, and smoking prior to the test
abnormal result is obtained on the glucose -Recommend the following if positive results and diagnosis of GDM is made:

-Ensure that food intake is adequate to prevent ketone formation and promote
weight gain
-Eat three meals a day plus three snacks to promote glycemic control
-40% of calories from good quality complex carbs
-35% calories from protein sources
-25% of calories from unsaturated fats
-Small frequent feedings throughouit the day are recommended
-Bedtime snacks are recommended
-Include protein and fat at each meal
Potential Complications Nursing Interventions
-Recommend dietary modifications along with weight loss
Macrosomia -Reinforce healthy lifestyle
fetal respiratory distress -Offer community based education
-Assess for s/s of complications at each prenatal visit
hypoglycemia -Check Blood pressure for changes and evaluate proteinuria when
polycythemia obtaining a urine specimen
-Measure the fundal height and review gestational age
hypocalcemia -Encourage woman to perform daily fetal movements to monitor for
hyperbilirubinemia fetal well being
-Prepare woman for the need for frquent lab and diagnostic testing to
congenital anomalies evaluate the fetal status.
-Assist with serial ultrasounds to monitor fetal growth and nonstress
tests and biophysical profiles to assess fetal well-being
