3. Secreties van het maagdarmstelsel

1. Speekselkliersecretie
- Hoeveelheid
 1-1,5 liter/dag
 Nuchter (interprandiaal of interdigestief): 0,25 ml/min via submandibulaire speekselklier.
 Stimulatie tijdens de maaltijd (prandiaal of digestief): 4 - 5 ml/min vooral via de parotis
speekselklier.
 90% door grote speekselklieren, 10% door kleinere speekselklieren verspreid in de
submucosa van de mondholte
 2 types van acinaire cellen:
 Sereuze acini (parotis): a-amylase
 Mucineuze acini (sublinguale): mucine glycoproteinen

- De speekselklieren vormen een heterogene groep met verschillende samenstelling van secreet
 gl. Sublingualis (3): muceus-seromuceus vocht
 gl. Submandibularis (2): seromuceus vocht
 gl. Parotis (1): waterig sereus vocht (a-amylase)
 kleinere muceuse klieren: muceus vocht (mucine)

- Functie van speeksel:
 Het doorslikken
 De smaaksensatie
 Mondhygiëne (wegwassen voedselpartikels, bactericiede werking (lysozyme, IgA), tandbescherming
 Verteringsfunctie (amylase)
 Beletten van uitdroging slijmvlies

- Vorming en samenstelling speeksel:
 Vocht en ionensecretie
 Actieve secretie t.h.v. de acini
 Modificatie t.h.v. de afvoerwegen
 Proteine secretie (sereus)
 Proteinen rijk aan proline: anti-microbieel
 a-amylase (ptyaline), nucleases, peroxidasen etc
 tandreiniging, smaaksensatie, vertering
 Mucus (sublinguaal en submandibulair)
 Smeren en voorkomen uitdroging slijmvlies
 Lysozyme en IgA (witte bloedcellen)  gaat ontwikkeling bacteriële flora in mond tegen
 Bloedgroepantigenen bij secretoren

- Werking:
 Aanmaak speekselvocht
 Parasympatisch zenuwstelsel: M3-cholinerg

 Orthosympatisch zenuwstelsel: a1- adrenerge stimulatie
 Peptide zenuwstelsel: purinerg (Substance P en ATP)
 Verhogen Ca2+.
 90% van totale speekselsecretie door grote speekselklieren
 Samenstelling verandert afh van de flow rate (snelheid van secretie)
 Laag: hypotoon (reabsorptie)
 Hoog: isotoon (beperkte absorptie capaciteit?)
