Beste lezer,

Wij zijn … en …, beiden vierdejaars HBO Verpleegkunde studenten aan Hogeschool Windesheim.
Momenteel volgen wij onze Bachelor stage op de dagbehandeling en verpleegafdeling van het
Oncologisch Centrum in het Isala in Zwolle.

Voor deze stage hebben wij de opdracht gekregen om een gezondheidsbevorderingsplan te
ontwikkelen op basis van een praktijkcasus. Daarvoor is een specifiek gezondheidsprobleem gekozen
waar we onderzoek naar zullen doen en we zullen passende interventies bedenken om de
gezondheid van de patiënten te verbeteren. Daarbij richten wij ons in dit
gezondheidsbevorderingsplan op angst door alle gevolgen die komen kijken bij de diagnose
borstkanker en het daarbij voorkomen van ineffectieve coping. Wegens de privacy (AVG-wet) hebben
wij alle patiëntgegevens in dit gezondheidsbevorderingsplan geanonimiseerd.

In dit verslag wordt eerst het gezondheidsprobleem uiteengezet, gevolgd door een analyse van de
gedragsdeterminanten en de daaruit voortvloeiende interventies. Tot slot zullen we de borging en
onze reflecties beschrijven.
