Hoofdstuk 4 - kinderdermatologie4.1.Eczeem
= verzamelnaam voor huidaandoeningen gekenmerkt door klinisch polymorf beeld. Jeuk is
meestal voornaamste klacht – krabletsels (kan secundaire infectie geven met pijn en koorts).
acute fase – erythreem, oedeem, papels en vesikels
chronische fase – schilfering, lichenificatie en evt. kloofvorming
o lichenificatie = verdikte huid, met toename van huidplooien en vergroving
huidstructuur.
Huiduitslag is vaak opvallend + cosmetisch storend geeft bij pubers soms psychische
problemen.
4.1.1. Seborroïsch eczeem
Eczema seborrhoicum infantum (berg) = kort na geboorte ontstaan en voor 6 maand
verdwijnt.
o Symptomen – vettige schilfers op behaarde hoofd en in lichaamsplooien
o Complicatie – secundaire infectie met candida albicans of soms bacteriën
o Behandeling – vette basiszalf- of olie (corticosteroïde bij hardnekkig geval)
Eczema seborrhoicu adultorum = na puberteit, chronisch recidiverend beloop en kan
tot hoge leeftijd voorkomen.
Gist Malassezia furfur
4.1.2. Atopisch eczeem (constitutioneel eczeem)
Meest voorkomende vorm = in volksmond dauwworm bij kinderen.
- Atopische constitutie die zich verder kan uiten in astma bronchiale, allergische rhinitis
en voedselallergie
- Oorzaak – niet gekend, momenteel uitgegaan van dat samenspel tussen erfelijke en
omgevingsinvloeden ledt tot verhoogde IgE-productie geeft heftige reactie op
omgevingsfactoren zoals voedsel- en inhalatieallergenen, uit zich in eczeem, astma of
rhinitis.
- Ontstaat vaak rond 3de maand, kan ook later
Drie leeftijdsfasen te onderscheiden
Zuigelingen fase – op wangen, voorhoofd, romp en extremiteiten (scheenbeen) staan
erythreem, sqamae, papels, vesikel en crustae – JEUK
o Narcosekapje (= gebied rond mond en neus) blijft vrij in gelaat.
Kinderfase (2-10 jaar) - lichenificatie met eczeem in elleboogplooin, knieholten, polsen,
enkels en hals.
Adolescentenfase – huid is droog tot extreem droog op handen, voeten en typische
predelictieplaatsen (buigzijde armen en benen) door verminderde talg- en
zweetsecretie.
o Eczeem verloopt exacerbaties (wintermaanden) en remissies
Behandeling – uitlokkende factoren vermijden zoals stress, transpiratie.
1. Bestrijding droge huid – bad met vettende badolie + insmeren met vetcrème
2. Behandeling eczeemplekken - corticosteroïden
3. Bestrijding jeuk – antihistaminica per os
4. Antimicrobiële therapie (secundaire infectie)
, 5. Dieet (voedingsallergie)
6. Topicale immunomodulatoren
4.1.3. Intertrigineus eczeem (intertrigo)
= erythreem in lichaamsplooien, niet scherp begrensd en pijnlijk gevoel + lichte
jeuk.
- Vooral bij zuigelingen, adipeuze patiënten en diabetici.
- Secundaire infectie met bacteriën of candida albicans
- Intertrigo voorkomen?
o Bij dagelijkse hygiëne alles goed spoelen + drogen
o Beperk inwerking van transpiratievocht – kledij verversen, seizoensgebonden
niet knellende kledij
o Huid beschermen met zinkzalf. Moeilijk te verwijderen met zeep, wel met
amandelolie!
4.1.4. Luiereczeem
- Ortho-ergisch contacteczeem = vorm van eczeem die berust op toxische reactie op
stoffen die huid irriteren (urine en ontlasting in luier).
o Symptomen – rood, geirriteerde en soms kapotte huid.
o Secundaire infectie – candida albicans.
Behandeling
- Kind regelmatig verschonen! Belang van zo kort mogelijk contact met urine / stg voor
kind
- Billetjes schoon met lauw water, géén zeep of lotiondoekjes
- Zinkzalf of zinkolie
- Regelmatig met tijdje billen bloot liggen
4.2.Huidinfecties
4.2.1. Bacteriële infecties
4.2.1.1. Impetigo
Meest voorkomende bacteriële huidinfectie
- Staphylococcus aureus en soms streptococcus pyogenes.
o Symptomen – blaasjes en blaren bij pasgeborene en zuigelingen (impetigo
bullosa)
o Symptomen – erosies en gele crustae bij oudere kind (impetigo crustosa of
krentenbaard)
- Huidinfectie treedt op op plaatsen waar huid beschadigd/kwetsbaar is (plooien,
luierstreek, navel of neus bij verkoude kinderen)
Behandeling
- Afhankelijk van uitgebreidheid letsels
o Beperkte impetigo – lokale behandeling met antimicrobiële zalf
o Uitgebreidere impetigo – oraal antibioticum
