Working phase: collaboration, therapeutic communication, empowerment,

goals, strategies. The nurse and the client collaborate together to meet the

clients health needs.

Termination phase: End of shift, transfer, discharge. Summarize care

requirements, plan and goals, progress. Termination can be tough.
