Home Health Care - ANSWER Skilled medical or rehab services in patient's home in order to promote,
maintain or restore health. Overseen by physician.



For Medicare Coverage - ANSWER Must be under care of physician and be certified for need of one or
more of following:

-intermittent skilled nursing.

-PT

-OT

-speech pathology

*pt must have had face to face with provider no more than 90 days prior to start of home health or
within 30 days of start.



Documentation of Face to Face Encounter - ANSWER Reason they were seen.

How home care relates.

Why homebound.

Physician signature.



Episode of Care - ANSWER Plan of care must be certified every 60 days.



Certification Requirements - ANSWER Physician must be medicare enrolled.

Physician must not have financial relationship with home health care agency.

Must estimate how much longer services will be required.



Limits of Home Health - ANSWER Meant to be for medically necessary short term care. Fewer than 7
days a week or fewer than eight hours a day for periods of 21 days or less.



Home Care vs Home Visits - ANSWER Patient doesn't have to be home bound. Interest in home visits is
increasing.
