Quarterly Progress
Ans- Completed with the MDS review. Documents overall progress and
response to programming for the previous quarter. Address any change in coding
on the MDS and the need to revise the activity care plan interventions in
response to these changes. The rational for the decision to revise or not revise
the care plan is documented in the progress.
Assessment
Ans- The process of discovering who the person you will be serving is and what
his/her physical and psychosocial needs are in terms of optimum care and
functional status

standard intervention should be used.
RAI
Ans- Comprehensive interdisciplinary assessment of the newly admitted
resident by day 14 do admission. Activities professional is responsible for
completion of MDS 3.0 Section F and, if triggered, the Activity CAA and the
information on the CAA Summary Form.
Care Plan
Ans- Starts on admission to address immediate needs. The guidelines requires
activities to participate in the baseline care plan completed within 48 hrs of
admission to begin the process.
Attendance
Ans- Daily documentation of activity attendance. Proof that activities were
offered to each resident. Bedside log to be used for room bounds residents
,reflecting type, frequency, length of visits and response to visits. Resident
refusals to be documented with each occurrence.
Change in Condition Progress Note
Ans- Completed with a change in level of activity participation or ability to
participate .Changes may be temporary, due to an illness like the flu, or may be
permanent. Progress note addresses the need for a revised activity plan and the
care plan is updated. Permanent changes due to a decline in physical, mental, or
psychical functioning are assessed by the interdisciplinary team for the need
for a new MDS.
Chart Note for Each Treatment Session
Ans- Recreational therapy to documents resident's performance and feelings
after each treatment session.
