Here is a scenario to consider: (Modified from Gabel, Rando & Melloh, 2016)
An 77-year-old female, Gwen, sustained a non-specific cerebrovascular accident within the left
hemisphere affecting lower limb strength, balance control, and difficulty with speech and
aphasia, Gwen is a retired nurse who worked at the local hospital for her entire career before
retiring at age 67 but continued to volunteer at the hospital until her stroke. Gwen would like to
resume volunteering at the hospital. Gwen lives with her husband of 50 years, Fran, who has
hypertension, and arthritis. Their son, Peter, lives about 15 minutes away and is concerns that
dad is sometimes forgetful which may impact his ability to provide care to mother if needed.
Over the last year, Gwen progressed through four stages of rehabilitation. These included:
(1) an initial one month of acute inpatient management;
(2) one-month inpatient rehabilitation at a specialized rehab centre;
(3) three months of transitional care at home to assist home skills for self-care and daily living;
(4) seven-months community support including through nursing and carers; five times per week
intensive outpatient rehabilitation involving hourly sessions of twice weekly occupational and
physical therapy group falls prevention, an individual physical therapy session providing balance
training, mobilization and massage with planning of additional home exercise.
